How to Enroll and Refer

Enrollment and Referral: Easy as 1-2-3


Speak with Brandman Centers for Senior Care (BCSC) by calling 818.774.8444, toll free at 855.774.8444 or via TTY at 818.774.3194 during the hours of 8:30 a.m. to 5:00 p.m., Monday through Friday. You will receive basic information about the program, a review of eligibility requirements, and an appointment for an in-home visit if you seem qualified.


Meet with the BCSC enrollment representative in the comfort of the senior's own home to discuss the program. The enrollment representative will obtain further information about you and your healthcare needs. During the visit, our representative will provide information on the following:

The enrollment representative will be glad to answer any questions you may have about the program.

If you are interested in taking the next step, the enrollment representative will provide a release form to sign to obtain the senior's medical records, as well as a statement of understanding about our presentation of the program. We will arrange a date for the senior to visit the Center for a comprehensive healthcare assessment.


As part of the assessment period at the Center — typically during two days — we will collect information on medical status, abilities, social and psychological needs, and current living arrangements. These needs will be reviewed by our Interdisciplinary Team of healthcare providers. The team will include an enrollment representative, the Center manager, a primary care physician, social worker, occupational, physical and speech therapists, recreation therapist, dietitian, personal care attendants, transportation coordinator, and any others who may provide care.

Within a week or two of assessment, the Interdisciplinary Team will meet to share their findings and determine whether or not the criteria are met for admission to the program. If so, an individualized plan of care will be developed. Successful enrollment is still subject to approval of the California Department of Health Care Services, to whom we will submit our findings.

As soon as approval is obtained, typically within five days, the BCSC social worker on the team will communicate what the Team has recommended, and set up a conference. At this time, the senior may enroll as a participant in BCSC.
