In this special community, known as the Brandman Centers for Senior Care (BCSC) PACE , each participant is valued and cherished. This program ensures that every senior’s unique health needs are met through a comprehensive approach with a team of interdisciplinary healthcare professionals. This caring team carefully assesses each participant’s individual medical requirements. For instance, if the patient is in need of rehabilitation services, appointments are promptly scheduled.
Rehabilitation Services, encompassing physical, occupational, and speech therapy, play a crucial role in enhancing the well-being of older adults. Within the PACE setting, these services offer specialized care tailored to enable elders to remain in their homes for an extended period, and in many cases, support them in living at home throughout their entire lives.
Physical Therapy focuses on enhancing strength, balance, and gait training essential for mobility. This service is provided both within the participant’s home and in the community. Occupational Therapy addresses the activities of daily living, encompassing tasks such as eating, dressing, toileting, and bathing. Speech Therapy is dedicated to cognitive and memory training, as well as addressing any swallowing difficulties that may impede their daily intake of foods.
At BCSC PACE, the team closely collaborates to establish personalized goals and outcome expectations for each participant – all aimed at maximizing the health and safety of our participants.
Our staff of Physical Therapists at BCSC PACE has the privilege of working closely with remarkable seniors as we tailor individual care plans to their unique needs. While not every participant requires physical therapy, we take pride in knowing that those who do receive the finest hands-on care from a team of professionals genuinely committed to enhancing quality of life.
