For Medical and Social Services Professionals
Using the innovative PACE (Program of All-Inclusive Care for the Elderly) model, the Brandman Centers for Senior Care enrolls seniors who are:
Age 55 or older
Who live in the BCSC service area
In need of a nursing home level-of-care as determined by the California Department of Health Care Services Long-Term Care Division
Want to stay in the community
Participants must be able to live independently at the time of enrollment without jeopardizing health and safety.
Participants' costs are covered by Medi-Cal, Medicare and private payment. Payment depends upon Medicare and Medi-Cal eligibility.
PACE has a record of success in supporting frail elders. Providers across the nation with a variety of sponsoring organizations have successfully replicated the PACE model.
Nationally, the typical PACE participant resembles the typical nursing home resident — an 80-year old person with seven or eight medical conditions and limitations in several activities of daily living. Nearly half of enrollees have been diagnosed with dementia. Nevertheless, more than 90% of PACE participants around the country can continue living in their communities.
